| 1 | Complete and sign the Authorization Statement that directs your bank to send all return items you receive directly to us. |

| 2 | Complete and sign the Electronic Services Agreement that authorizes us to convert NSF checks into electronic transactions. |
| 3 | Submit the signed agreements and a voided business check. A representative will handle all the paperwork with the bank and our office. |
| 4 | Display the Notification Stickers and/or utilize the stamp on customer checks. We will notify you on receipt and recovery of NSF checks. |
